My 91 9kT gets this periodically, in fact is doing it now. What has been helpful in reducing it has been cleaning the automatic idle control, and then repeating the process a couple of times over a two week period.
Cleaning procedure is to take the big air intake hose off the trottle body housing, then squirt a generous amount of carburetor or throttle-body cleaner right into the AIC hole (you'll see it in the bottom of the t-b.) I use the little red tube that comes with the pressure-can of cleaner to get the dose where I want it. Once having squirted, I move as quickly as I am able to close the t-b by getting the air intake hose back in place (I also DO NOT BREATH THAT CLEANER--it's 'way toxic!)
Once the system is reclosed, I get in and start her up. Some smoke, not much, then go for a drive. This all leads to smoother running.
Following up with repeats a couple of times in succeeding weeks seems to lead to a year or so of improved idling.
I don't know of a downside to this approach, but would be glad to hear input.
