Your low side pressure indicates that something is causing refrigerant starvation in the evaporator. The expansion valve is the most obvious suspect. The question is what is it doing, and why.
I am no expert in R12, but I thought the high side pressures with r12 were a lot lower than r134a. I suspect from your initial low side pressure of 26-28 that the pressures were taken at engine idle speed. If you are getting 240 psi on the high side at idle on an 80 degree day, that would be high even for r134a. If 240 is the pressure at 1500 and it's 90, then that would be ok for 134a, but may still be high for r12.
My 91T is on it's third expansion valve of the season and things are finally working right this year. Number 2 was probably ok also, but by then I was so frustrated I replaced it along with the drier. Quasi got one for his 91 from airpartsonline that was defective. Where did you get yours? The one I am using came from eeuroparts. If you are going to replace the valve, then replace the drier also. And flush again.
