Dan, I happen to have the DI/APC manual for the 91T, which I bought to rectify a similar problem. The manual is "Ordering No." 364091 and I highly recommend it.
First, the manual says to do the adaption run, you pull fuse 23 (5amp) for at least 10 seconds to reset the ECU. Next you peel back the boot on the throttle switch (don't disconnect it), and bridge pins 2 & 3 from the back. Then run Wide Open Throttle (WOT) from 2,000 to 3,000 rpms for more than 3 seconds on a big hill.
If that doesn't fix your problem, you may well have a sticking BPC valve, a loose vacuum hose, a leaking hooter valve or engine knock. My problem was the BPC valve, which the manual indicates is pretty much the most common failure. Also, there is an actual knock sensor on the back of the block that is sensitive to dirt in the electrical connector.
