Correct me if I've misread your post, but it sounds like the tranny shifts okay once you start rolling by manually downshifting to first gear. If this is the case, it is your first one-way clutch (aka "freewheel"). By way of explantion, there are parallel circuits in the fluid system for shifting into first gear. One of them (the automatic downshift one)involves the freewheel. The freewheel engages when the motor is turning faster than the input shaft of the transmission (i.e., when the car is accelerating). The manual circuit bypasses the freewheel and forces the pneumatic clutch to engage directly.
If this is the problem, you should be able to shift into first at a stop, accelerate to about 17 mph and shift into "D" and the transmission will behave normally.
Also, when you drain your transmission fluild, you might see thin accordion-shaped pieces on the magnet. They are the springs for the freewheel.
Unfortunately, if this is your problem, the transmission needs to be completely disassembled to repair the problem. However, you can drive shifting manually into first at stops for a long while unless the metal migrates to other areas of the tranmission.
