The piston is sticking and not retracting easily. This causes the brake pads to drag, which heats up the piston. This causes it to stick more, and warp the rotor, which can cause that judder at speed, even with your foot off the brake. I've had this happen to me.
This can be due to grit stuck between the piston and bore, corrosion between the piston and bore, or a combination. You can flush the fluid from now to the end of the year and you won't clean out piston/bore issues.
The inexpensive approach is to remove the piston, buy a rebuild kit, and rebuild the caliper. I've taken the easy way out - I simply buy a rebuilt caliper and trade in the old one. It cost me about $80 from my local autoparts store the last time I did this. More expensive than a rebuild, but it gets the car back on the road again. And if the caliper is really badly shot, I'll put the time and effort (and $$) into the rebuild, and still have to replace it.
