The problem appears resolved.
I pulled out the plugs for inspection. I noticed that the plugs all seemed loose. Not spinning freely, but requiring less effort than usual to break loose. They were in generally good condition except that there was significantly more carbon than I typically see. The only anomoly was #4 which smelled more distinctly of fuel than the others. The carbon brushed off easily and revealed electrodes which seemed almost new looking. I checked the gap and adjusted a couple slightly so that they are all at about .9mm. I also pulled out the entire rubber tube between the air filter and the throttle body. I checked the air filter which also looked almost new. I cleaned the AMM by spraying it with electrical contact cleaner. I did not observe any dirt on the element in the AMM before I sprayed it.
I put it back together and the car started and has run fine. I am curious about the carbon build up. It could be the result of the outrageously cold winter and my short commute work. I wonder, though, if it could indicate a problem with the mixture arising from the O2 sensor or AMM.
