Engine running rich - check engine light on

The care is a '91 9K CD Turbo with 172K miles on it. The engine is running rich with sooty deposits on the plugs and the piston heads. Check engine light is on. Air and fuel filters are new. ECU codes report "mixture too lean / too rich, Oxygen sensor unable to regulate". Oxygen sensor is only one year old.
Has anyone had such an experience? If so what was the solution. Has anybody had a Air Mass Meter failure that caused this?
