No faults on the Self Test says that the servo is OK. The self test checks to see if the servo moves, it doesn't check to see if the flap moves.
Since you can move the valve by hand, the problem is pretty obvious - the linkage between the servo and flap has fallen off. The servo is a motor and geartrain - you can't move the servo by hand. Therefore, since you can move the flap, the flap isn't connected to the servo any more. The servo has a shaft, and there is a metal arm on it held on with a set screw. At the end of the arm is a stiff piece of wire (piano wire) held on with a clip. That wire comes through the firewall, and connects to an arm on the flap.
You should be able to see that wire from the engine compartment side, and see if it's connected to the flap. If it isn't, you've found your problem. If you can pull and push the wire with no problem, then the problem lies in side the dash, and it's as simple as something just fell off. Either the clip on the servo arm, or the setscrew let go.
