I'm in the process of finishing up a tranny replacement in a 91 9000S.
When I put the transmission in, everything bolted up correctly, but I couldn't get the intermediate driveshaft to fit the way it was supposed to. Turns out that the bracket that bolts onto the engine block, rests on motor mount, and holds RH inner CV joint had cracked. In my case, the bracket cracked in such a way that the CV joint was in one half, and the other half remained bolted to the engine.
What caused it? In my case, I suspect an accident by the PO. I bought this car for $500, and discovered several things that have been done that point towards an accident (ATF cooler eliminated by running hoses together, which burned tranny!).
In your case, I'm not sure. A possibility is that the intermediate driveshaft was not 100% in place and allowed excess vibration to be transferred to that bracket. They also could have stressed the bracket via pulling on the intermediate driveshaft when removing the tranny. The bracket isn't exactly a "wear item," but doing a search of the archives has shown it has happened to others before. I'd try and make a case to the shop--it seems possible that they could be at fault.
I do know, however, that the part number you gave has since been replaced by part #4387528. That informationn comes from the dealer ($172!!!). However, my new bracket is coming from Goldwing in NY--$85 + shipping.
