I've run into this before. If I've gotten to the CV joint early, when it still has most of it's grease, and more importantly, dirt hasn't gotten in, I've been able to replace just the boot, and the CV joint has been fine. This has worked for small tears, and again, gotten to early. How do you know early? There's still a decent amount of grease in there, and the grease flung out doesn't look old.
However, if the joint was run with minimal grease, or it looks contaminated by road dirt, I'd replace the joint. In order to replace the boot, you've got to pull it apart - you've got to separate the joint from the hub. The additional labor to replace the joint is minimal (as Yaofeng says). The cost is for a new joint / axle. If you try and go cheap, you'll end up doing the job twice.
It is a judgement call - a small tear, and plenty of grease inside, I replace just the boot and all has been well. If I have any doubts about how long the joint has been compromised, I just replace it. I hate doing things twice.
