More precisely, +12 AND ground.
One side of the siren is tied to power all the time. That won't cause the alarm to sound until the low side of the siren is connected to ground. The low side of the siren (black wire) connects to pin 11 on the alarm module. When the alarm module decides to sound the siren, it connects the low side to ground.

If the siren isn't making noise when it should, and you see the 12 volts going to 0 (or near that) when the alarm should sound, then there is a poor connection between the hot side of the siren and the power source. That poor connection is a resistance. With no current flow (siren off), there is no voltage drop. But when the low side of the siren is grounded, the current draw goes way up, and there is a big drop across the resistance. So the 12 volts you see drops down, so far down that the siren won't sound.

Check the wiring/connections from the hot side of the siren to the fuse (#16, I think). If you want to be sure, simply run another wire from +12 to the hot side of the siren, trigger the alarm, and see if it sounds. One way to do that is to hit the autoparts store and pick up an in-line fuse holder and a 15 amp fuse. Tie one end to the hot side of the siren, and touch the other to the + post of the battery. Why an in-line fuse? because if you just used a piece of wire, and accidently touched it to ground, you'd arc out the battery, which could cause an explosion and burns. Not pretty.
