The cap appeared to be fixed, with no moving parts, which is what was mystifying. But based on your explanation, I inserted the shaft of a small screwdriver into the hole in the top of the cap and pushed hard until the plug/piston and spring popped out of the bottom of the cap. Apparently, rubber had flowed into the well of the cap which caused the piston to be stuck fully compressed.
So I guess the lesson for all is, not only should you replace the rubber diaphram but also make sure the piston/spring is working. If I had just put a piece of rubber in, the valve would still not work correctly, it would have been stuck open.
