The O2 sensor operates in a very narrow range of mixture. If something is causing the engine to run too rich or too lean, the O2 sensor will be out of range, and the ECU will throw an O2 sensor code.
Check the plugs - they live in the engine and tell all. If you're running lean (most likely), I'd look for vacuum leaks. Notice the plural there. Just finding one doesn' necessarily mean you're done. Look for cracked and broken hoses, or just dangling ones. My favorite leak is the charcoal canister, because you can't see it. Pop off the right front turn signal and look in - you'll be looking at the top of the canister. Make sure the hoses are attached. Make sure the diaphram on the Hooter valve (turbo bypass valve) is OK. Other causes of lean running would be an old/clogged fuel filter, and possibly a bad Fuel Pressure regulator (FPR), depending on how it's failed.
Check to see if just one cylinder seems to be running lean. There I'd suspect a lazy fuel injector.
If you're running rich in all cylinders, first I'd suspect a leaky FPR. They can leak internally - suck on the thin hose from the FPR to the intake manifold - if you taste gas, it's probably leaking (it normally tastes awful anyway). Just one or two cylinder - sticky fuel injectors. A weak or faulty MAP sensor can also cause rich or lean running.
