...the fact that you found loose nuts on the bearing is odd. Maybe it had some work in that area sometime in the past?
Do you suppose it could have come back from a snapped timing chain in its past? See any signs of flying metal in the timing gear area? I ask because I replaced a piston in our 92 after it got a piece of valve guide embedded in its crown during a timing chain event. If that scenario pans out, then perhaps the mechanic didn't properly tighten the nuts.
What else? I suppose if there was sufficient pinging over time that might have stressed the rod, or maybe even a lubrication issue? Lube seems unlikely, although from 94 on these engines get a little more susceptible to sludging.
After thinking while I type, I guess I'd have to guess one or multiple severe pinging episodes must have caused a fracture, which finally failed catastrophically. Or at least, that sounds good.
