Probably the blower motor -- controller may fry again

Just hooking up 12V and getting the fan to run does not remove the blower motor as a (probable) culprit.
When the blower motors gets old, things start to stiffen up (i.e., bearings) and although the motor will still turn, it requires higher and higher current to do so. This high current ultimately cooks the fan speed controller.
If you really want to permanently fix your blower motor, and not run the relatively high risk of frying successive controllers (which cost upwards of $150 each new), then unfortunately you have no other recourse than to replace the blower motor.
Pity, when the heater core was done, that the motor wasn't also done at the same time. That's a classic 9000 precaution, to save you having to go into the same place and repeat the job.
If you want to try the controller first, there is a transistor in there that you can source and solder in yourself, for something like ten bucks, but I've only had about 50% success with that option myself. Or perhaps you can find a good controller from a boneyard or Ebay.
It is a mystery why the blower fuse does not trip when this high-current situation arises. Apparently it's there for show!
