Everything you're seeing is likely a bad battery. The voltage when the engine is running is just fine - mid 13's, so the alternator is OK. Leave it be.
This battery has been discharged a bunch of times. Each time a battery gets fully discharged, it loses life. Three years and a few discharges, and the battery is probably shot. What happens is that the plates get sulfated up - they lose surface area. That doesn't impact the no load voltage, but it does impact the current capability. It may seem charged, but it is like you stuck 8 AAA cells in there - you'll see 12 volts with no load, but a few AAA cells won't start a car.
When you first turned the key to ON, you read the battery voltage of 12 (the alternator doesn't produce voltage until the engine is turning). Not unreasonable for a battery with minimal electrical load.
When you did mange to start the car, the voltage dropped WAY down. That killed the always on power bus to the radio and clock. They can work with pretty low voltages, but the battery went down below that. The same as removing and reinstalling the battery.
A classic weak battery will appear to charge up very quickly, drive just fine, and then lose charge very quickly - overnight, or a matter of hours. The only fix is replacement. Once the plates get sulfated, they stay that way.
