I had almost the same identical problem. No heat because the servo motor did not move the temp flap door. Door arm was fine and the wire was not binding. Would run the ACC through diagnostics and 90% of the time it came up with no fault code. Other 10% of the time it would show one fault code (when the arm would not move). I went through all of the temp sensors, checking resistance per the service bulletins from the Townsend site. They all checked out fine. Last weekend I finally replaced the entire servo motors, bracket, and mixed air temp sensor assembly with one I got on ebay (cheap). Has been working fine ever since.
I think I either had an intermittant problem with the servo motor or for the mixed air temp sensor.
You will have to remove the passengers side air bag and the top of the dash to replace the servo and mixed air sensor assembly. I had to use a flexible drive with the small torx socket to get the one bracket screw closest to the firewall out.
