The parts link above will work for 1992-1998 cars. And since it is the ACC system, there is no switch to fail.
It would be good to know what you mean by intermittent. The electronic controller does fail, but the usual failure mode is the fan is on full blast all the time. It would be a shame to replace a $150 part and find out that's not the problem. It sounds like you hooked the fan up to 12 volts directly, in which case it should blast air. That indeed is a test of the fan, but not a perfect one. A fan that has a lot of drag (e.g. bad bearings) will blow fine at full power, but may not work so well at lower power settings.
Can you be more specific about the 'intermittent' problem? Does it just stop blowing air? If so, if you manually increase the fan speed, does it simply not increase?
