Not exactly on point, but my '96 CS auto suddenly lost fourth gear last year. I have to put it in D3.
My problem is, when it goes to make the 3-4 upshift, it shifts into neutral instead - - out of 3, but there's no 4th gear there.
I have done lots of reading on here and recently talked to Nat at Eriksson Industries, the recognized top shop for these trannies:
He said 90% of the time my problem happens, it's a cracked "Belleville spring," a dished spring washer that sits at back of the linkage that applies and dis-applies the C brake band, the friction element that does the shift to 4.
He said sometimes a pie-shaped piece cracks out of it and gets in the way of things.
On older 9000s there is an elaborate way to remove and replace this assembly in-car he said, but those with ABS, too much in the way.
I have seen a pretty good picture of the C-Band assembly on the PDF recently posted on this site by, was it, Bill Davies, from the 9000 supplement manual published by the Automatic Transmission Service Group.
But your problem, shifting instead to a lower gear, and affecting not just 4 but 3 as well, doesn't seem to be what my problem is.
My suggestion would be to look up Eriksson and talk with them about the problem. It could be yours is some problem in the valve body, which CAN be removed and have work done on it with tranny still in car. Or for a pretty high price, a rebuilt valve body can be bought, from Eriksson, and swapped in.
That mystery loose washer could be a clue. Does your shop still have it?
