I have seen (from personal experience) where reverse is fine, but all the forward gears go away. In my case, the car exhibited some slipping when accelerating from a stop. I was waiting at a light, and when I hit the gas, the car didn't go forward. No muss, no fuss, no bangs, bumps, and no movement. I got the car off the road with reverse only (on a busy street, THAT was exciting).
In my case, fluid levels were fine, but burnt. When the autobox was opened, the clutches were burned up, and all the teeth broken off one of the (fill in the blanks, not sure of the exact name). A complete rebuild was in order, alas.
The autobox on the 9000 is notably weak, and they die. You can do everything right, and they die. They tend to last longer if you maintain them well, but that is no guarantee.
It isn't bad luck - it's a weak design. I expect 80 -120K miles from them. Some more, some less.
