I hope others have better news for you, but my experience with my '87 Turbo 16 at about 190,000+ miles was that one day, out of the blue, it jumped out of 5th. After this, it would go into 5th OK, but the slightest touch of the throttle would make it jump out again. The failure was as sudden as that.
I took it to a local transmission specialist, who came highly recommended, and they diagnosed a worn selector hub. The only solution was to take the transmission out and either replace the hub or replace the transmission.
They asked me if I had noticed the gear lever "kicking back" under acceleration in 5th. Indeed I had noticed this even when I bought the car at 168,000 miles. This is apparently a classic symptom of selector hub wear - eventually it kicks back so far as to jump out of gear. If yours does this in 4th, I would suspect a similar fault. I'm told that the early 5-speed transmission is, on average, good for about 200,000 miles.
I drove my car beyond the 200,000 mile mark without 5th gear, but of course 4th is harder to do without.
