I have worked on many cars and their brake systems. If you look at the design of the Calipers on Disc Brakes, It's more or less impossible for that to happen. There is no way for the pad to fall out unless it is Damaged some how. Like broken in half and even then it's not likeley to happen. If the Caliper is damaged, Then, I could see it happening.
I mean, You look at the Clearances between the Wheel and the Caliper, You look at the Clearances between the Rotor and Caliper and you'll see what I mean, There is no way for the Pad to fall out, Especially if it is undamaged and still has some meat on it.
Was it the right Pad for the Vehicle?
I'd like to know more about this "Phenomenon". It sure is weird..
