If you apply 12 volts to the horn, does it sound? If not, check the ground connection, and if that's good, replace the horn. Otherwise, start working backwards from the horn. Actuate the relay - does the horn sound? As you work backwards, at some point the horn won't work, and the problem lies between there and where you last checked.
This method annoys the neighbors (horn sounding as you test), but it is basic troubleshooting. But always start at the end (horn, light bulb you're trying to light, etc), and work backwards.
