The radio gets two sources of power - one is switched with the key, and the other is there all the time. The 'all the time' power keeps the radio presets alive, and also lets the radio know if it's been removed. What's happening is that the 'all the time' power is being interrupted to the radio, so it figures it's been removed, and asks for the code.
If you're just driving along and suddenly the radio resets and asks for a code, and nothing else happens, then I'd suspect a loose connection, either where the radio plugs in, or worse, behind the radio itself in the harnessing. Remove and plug in the radio a few times, to see if that helps.
If the radio asks for a code only after you've started the car, not while driving along, I'd suspect a bad connection near the battery. The voltage drops pretty low during starting, and if there are any poor connections, especially grounds, the voltage may drop low enough for the radio to think it's been disconnected, but the car still start. This isn't a battery issue - it can be the battery clamps, cables, or where they attach to the rest of the car. Peek into the battery cables where they meet the clamps - is the copper wire clean, or is it caked with a whitish or greenish powder? That's corrosion from the battery acid fumes. If the cables are gunked up, think about replacing them.
