Quick check- grab the balance shaft (BS) pulley on the front of the engine and check for play. This will tell you if the BS front bearings are shot. These are the ones that normally go, as the tension from the fan belt puts additional stress on the front.
The BS comes out the back of the motor, so no chance of replacing without motor removal. The bearings can be replaced by the normal home mechanic. I have a nice paper written up some 20 years ago that tells you how. I've done it twice.
