Clutch Actuator Arm broke off!

Hey there...if you can remember I was having problems with my clutch a few weeks ago. The pedal went straight to the foor, with no engagement. I took it to a local mechanic who said that the actuator arm was loose or something and that he could weld it back together. Well, it worked great for about a week, then my clutch got progressively worse. It started engaging near the floor. It was at the point where it wouldn't shift into gear, and I barely made it home. Well, I figured that the clutch cable self adjuster had gone bad, so I just ordered a cable and replaced it. I put the new cable on...clutch feels great the first time I press it in..then I press it again..and the exact same thing happens..it goes nearly to the floor! Sooo frustrated! Note that I had to push the actuator arm to get the cable to slip into the little cradle thing. Now there is like 3-4 inches of slack on the cable by the arm..I tried yanking the adjuster to no avail. Then as I'm moving the clutch arm...I can feel that the weld that the mechanic did isn't holding up..finally the arm just snaps off at the point where it enters the clutch housing. My question is, what is the deal here. Do I have two seperate problems? One with the bad actuator arm, and one with some clutch release things? Or is it just that the arm needs replaced?? The clutch worked great right after it was welded? What is the name/part number of the "actuator arm"(I couldn't find anything on eEuroParts.com), how much does it cost, and how hard is it to put on?? Does the tranny need pulled? Thanks for any help in advance!
