I would not leap from any idle issue to any cause without knowing that the plugs were not a factor.

How do the plugs look?

Always used synthetic?

Low oil pressure warning circuit is functional?

You might have some valve lifter sticking. That can occur with a group III synthetic. It happened to me with Castrol FakeTec(tm). Solvent type drive in the oil type cleaner addatives helped. Problem went away with Mobil 1, then Delvac 1 and never returned.

Some engine noises are better heard in the cab, fan off, hood and windows closed and only at idle.

75K seems too soon for any chain problems if the oil has been taken care of.

If most of the AC gas leaks out, a pressure switch will not allow the compressor to engage, which then makes it impossible to have any effect on idle etc.
