There are two wires going into the cig lighter, but only one is the 12V supply. The other is to light up the dash at night. The 12V comes straight out the middle of the cig lighter; the bulb comes out off to the side.
My modulator had a connector, but I cut it off and used a wiretap (18-22 gage wire from Radio Shack, see link). The wiretap has the same effect as soldering the 2 wires together, but only takes 30 seconds to do. Basically, I took the modulator 12 V wire put it in the tap, found the lighter 12V wire. Slipped the tap over the wire and used pliers to close the tap. As the tap closes a metal bracket slides over the wires and strips the outer cover of the wire. The metal bracket provides the contact between the two wires. For the ground I connected it to the vehicle frame. When the glove box comes off, there are two bolts near the passenger door. These bolts are used as a ground for the ECU, so I backed out one of the nuts and slid the modulators ground in. Using the tap still allows me to use the cig lighter for other electronics (cell phone, etc.).
