Ian, with that many miles, have you ever cleaned the Idle Air Control valve? Search teh BB and there is a DIY set of instructions, it takes about 30 minutes, you may want to clean the Throttle body at the same time.
At lower RPMs any gunk in the IAC will cause it to stick and run rough.
I just cleaned mine at 50K and it was already getting black inside.
While you are doing that, be sure to check around and make sure all the vac hose are good, no cracks.
