The 4th gear has gone AWOL.
The transmission computer is preprogrammed to recognize certain transmission speeds at certain vehicly speeds when a certain gear is engaged.
Since the computer knows that it has choosen to shift into 4th and the car is going at the proper rate but the transmission is only spinning in third gear speed, the code is triggered.
When stopping the car and recycling the key, the transmission computer is reset to zero again but the code is still lingering hidden if you hook up a scanner.
As long as you drive slow enough not to trigger the shift into 4th gear you can drive all day long without triggering the code or limp home mode.
If you move gear shifter down to third gear you can also drive all day long without trggering limp home since the transmission is not trying to shift into 4th.
I had a Volvo S80 in the shop a couple of month ago with exact same problem and similar Volvo fault code for same scenario. As long as I drove slow enough it was ok. Or if I recycled the key after triggering limp home, the transmission worked flawlessly until trying to find the non exixting 4th gear again.
