inner tie rods

How difficult is it to replace the inner tie rods? I was at the shop yesterday trying to get my 1998 NG900S aligned. They could not turn the adjustment sleeve on right side. The mechanic told me it was badly stretched. I want to replace the adjustment sleeves on both sides along with outer tie rod ends -- they have torn boots. Is it worth it to replace the inner tie rods at the same time? They are somewhat expensive and seem to be fine. My steering is actually very tight. I recently replaced a worn wheel bearing, both lower control arms, anti-roll bar bushings and strut inserts. The car feels like new.
If I decide to change the inner tie rods, do I need to replace the lock plate and bushings that hold it to the rack? See picture -- that's the part I am talking about.
