The tensioner is a two part device where one part is solid mounted to the engine and the other is the swivel part that tension the belt.
Between the two pieces is a thin nylon disc (like a load bearing) and due to the steady load in one direction and that the outer part is constantly grinding back and forth on the nylon disc, the disc wear out and make the outer part with the pulley tilt slightly and that will make the belt slwly wander off the pulley.
So if the belt is already hanging "off the lip" you should be acreful since it will wear down the belt quicker and it could shred.
