Clutch Problems

My '98 NG900 has a problem with the clutch slipping. Actually it's more like it dis-engages and then re-engages. It usually does it in the higher gears, but it's not consistant. Sometimes it does it at fairly light throttle, other times I can accelerate at full throttle and it doesn't do it at all. It's been doing this for a year and a half and hasn't gotten any better or worse. My original thought was it's simply a bad clutch. The car has 140K on it so I doubt it's the original clutch. Assumming it was replaced at 80-100K it shouldn't be worn out. Could an original clutch last that long? I'm wordering if it could be a leaking rear main, weak pressure plate, or possibly something to do with the hydralic mechanism. The fluid was really dirty so I sucked it all out and replaced it twice--didn't beed it though. It seemed a little better for a while. Anybody have any ideas?
