replace mine. Was doing the rear brakes the other day, and although the pads are still ok, the pads material is starting to separate from the backing metal.

The adjuster adjusts the spacing of the bottom of the pads to the drum area and the cable adjustments pulls the top of the pad tighter to the drum area. You basically want the bottom as close to the drum area as possible without seizing the drum, then adjust the cable so when you pull the lever, it pulls the top tight against the drum. If the bottom adjuster is too tight, you drag the pads and can have premature wear, overheating, poor mileage, etc. If the bottom adjuster is too loose, the top won't have enough "travel" to properly grab on to the drum or the when it grabs, too little surface grabbing/holding the drum.
