Were you able to rule a malfunction in the Twice module (under the driver's seat)? I understand that a malfunction in this module can result in unusual and sometimes unexplained behavior.
The ignition switch would be worth trouble shooting in your situation. The lights are responding just as they would if you had removed the key (with the light switch remaining in the far right position) In other words: After removing the key the dash lights would go dark and the headlight would turn off. Perhaps the contact in the ignition switch is not registering the presence of the key (or the Twice module is not picking up or responding to the contact?) Keep us posted on this most interesting troubleshoot.
