If the idle is fluctuating up and down, goes up, goes down and stumbles, races up again... Try finding the throttle stop adjustment. Put a layer of something in there to keep it open a bit more, try paper etc. If this works, then examine how you can make a permanent adjustment change. The throttle flow may be such than the IAC and feedbackloop can't manage things. That happened to a few others, perhaps they were all turbo, can't recall that.
Don't know about the shifting. Exhaust leak not a problem unless the leak impinges in the O2 sensor(s).
Make sure that the O-ring where the plastic plenum fits the TB is located properly.
Some places will read your CEL for free.
