The A/C compressor and rad fac can kick in together, or just the rad fan. This puts a load on the engine via the compressor or alternator or both. The RPMs drop a little and the ECU opens the IAC to recover and you get and engine vibration surge. The thing that you hear might be relays for the compressor and fan. Most of this is normal, but is yours extreme? Hard to know via posts.
Is your battery old?
