If you look at the end that goes into the hose, you'll see that it doesn't taper down to match the inside diameter of the hose. Instead, there's a big lip on it, maybe an 1/8" thick. All people have done is grind that lip down, so as the air travels through the hose to the metal casting, there's nothing to interupt the airflow.
