I'm a book designer...these rivers of white space are the result of careless design. A thoughtful designer is always aware of the page's negative space, and will try to avoid rivers by changing column width, text justification, font size, or even the typeface itself. Sometimes just a single well-placed carriage return will do wonders for your alignment.
Some people...people who have better things to do with their time, I suspect...don't really notice things like insensitive typesetting, hyphen stacks, headings that are not in proportion, display faces that clash with their text faces, or my favorite faux pas, too many fonts going on. However for those of us who specialize in such minutiae, these are sins that identify the designer as an amateur.
