I'm not from a dealership or garage, but I do know that the garage I visit often fixes more than twenty cars a day, and it's not a big place. I'm sure they do not have 20 loaners. Since it is 15 miles away, I usually just have a friend or my wife bring me home from there or drop me off when the car is ready.
You could probably get a loaner if you asked for it when you made your appointment well in advance of your visit. And if some people need to leave their cars overnight (like for an early morning appointment schedule, that's even more loaners tied up for the day in uncertainty. Big dealerships probably service in excess of 20 cars a day easily.
The other issue you might be facing is that perhaps they don't have enough insurance or dealer/loaner plates to provide a larger number of loaners.
The big car rental companies are owned by the car manufacturers, so I'm sure they get a better deal than you, and the rental company probably doesn't want cars collecting dust.
