1) More than likely just backed up water. Can flood the whole right side of the car when really bad and when it rains hard.
2) Smaller hose drains a tray installed under the evaporator to collect any condensation drippings. Rarely a problem.

3) Part of any service at my shop. I have installed screen wire over the opening before, using body 'bondo' to hold it on three sides and a clip on the fourth to allow access. A little care every now and then keeps it clear without the screen though.
