There is a metal pin (officially, the "gear shift lever latch stud") located at the bottom of your shifter that prevents the shifter from going over to reverse without lifting the ring beneath the shift knob. You need to remove the shifter and adjust this pin by screwing it out - Bentley manual says it should protrude 22mm.

In the meantime, when shifting to 5th, don't think of the motion as UP out of fourth, DIRECTLY RIGHT beneath 5th, then STRAIGHT UP into 5th. Instead, think of the motion as more of a fluid, diagonal move, sort of like thinking of 4th as 7 o'clock and 5th as 1 o'clock on a watch. You want to go smoothly from 7 to 1. Takes a bit of practice, but I got good enough at it quickly enough that I didn't bother fixing the problem for 2 years.
