My guess is that the Hall effect is not working and that you are not seeing a tach needle bounce when you crank. The ground wire on the dist goes to a noise suppressor capacitor on the coil, not needed to get the car running.
I'm assuming that the ground wires on the head are all connected? These are the grounds for the injection, ECU and what else???
Have you tried the hall test outlined in Bentley. I did this test several years ago and from memory, it invoves turning the engine over and watching for the voltage fluctuation at (I think?) the Hall connector on the side of the distributor or the other end at the ignition module. you might be able to do this test by just turning a loose distributor by hand.
