forgotten washers. Washers spread force over a greater area, thus preventing breakage. Focused forces are concentrated so they break things like the pointy side of a nail through wood. If the system was designed for washers, (I can't recall as I did mine with my engine so alot was goin' on), then without them you have a focussed force rather than a spread out one.
Overtightenning will crush steel and damage what is being squashed. This is why I disagree with Trog's statements thast torque wrenches are not necessary. They prevent damage from overtightenning. Washers also protect somewhat. Usually the head of the bolt will just sheer off, but ya' never know. Perhaps pulley was not on flat, so tightenning it down bent and stressed it. Perhaps it was just an old or defective pulley. Perhaps your belts were too tight. They should flex a bit after tightenning, about 3/4" to 1 inch when warm.
