I assume that by 10 degrees, you mean of cam sprocket rotation , not engine rotation. 10 degrees of engine rotation would be a lot.
It's probably not very worn.
Tough call. How long do you expect to keep the car? What kind of shape is it in? How noisy has it been? If you're looking for another 100K miles, and the rest of the car looks like it'll make it, then the investment of maintenance is always good. The existing chain may very well be good for quite a while more - I'm not trying to say that this one is shot. It's just that the ideal is perfect alignment of the marks.
You can just replace the chain by rolling in a new one - a relatively cheap job ($500 or so). I'm not a big fan of this because if the chain is worn, so are the guides and sprockets. That's fixing only one thing. A full-up timing chain replacement is more on the order of $1K. But if you plan on keeping the car a while, it would be a good time to do some serious servicing - swap out the water pump while the timing chain cover is off, replace the main seal and oil-pump O-rings, check out the harmonic balancer while you're at it. This can drive the price up, but it'll be cheaper to do it all at once than spread over a year or two.
Again - judge the car. If you want another 5 years out of it, putting a few $$ in now is a wise investment. If you're tired of this car, and are looking to dump it in a year or so, then you make different maintenance decisions. I hate to say that because it implies passing problems onto someone else, but that's the real world.
