12223 - Air/fuel mixture too lean
Insure no vacuum leaks. Check injector seals. Check ELCD operation Check fuel pressure Check for code 12225
12224 - Air/fuel mixture too rich
See above
12225 - O2 sensor problem (Fault must be present for 3 minutes to set code)
Check sensor output(fluctuations between 0 and 1 volt) check preheater circuit and fuse
12111 - Incorrect air/fuel mixture while driving
Check for vacuum leaks. Check 02 preheater. Check ELCD system. Check fuel pressure
12112 - Incorrect air/fuel mixture at idle
See above

You'll want to eliminate the AMM as a suspect because, like JohnK said, it can cause the mixture faults and the 12225 fault. Unplug it, start it, and see what happens. If it runs the same as without unplugging it, then it's a good suspect. BTW, you'll get a sustained CE light immediately when you turn the key on if the AMM isn't plugged in properly.
You'll want to check the two other wires on the O2 sensor (The two wires of the same color that I described before). They both go to a connector by the spring tower different than the one I described before.
To eliminate the possibility of the O2 sensor preheter circuit from being bad, check the fuse for the Oxygen Sensor preheater. The fuses are labelled pretty well on the cover.
The more I think about it, the more I think that they just didn't plug something back in.
