If you are very lucky, you might be able to fish the grommet out of the throttle body from the front. I'd pull off the big hose and get a flashlight. If you can see it you're in luck. Even if you can't see it, maybe a vacuum cleaner might move it enough to be visible. If it's stuck up in the runners and you can't find it, I think you really should pull the intake manifold off. Not really that bad a job. Get a new gasket and while you're at it I would put in new injector orings and you probably should replace the copper washers on the fuel rail banjo bolts.
Base boost mystery has me puzzled. I would check all your turbo plumbing for hidden cracks and make sure the APC computer is well connected. Also make sure all your turbo flanges and wastegate mounts are tight. I think it's something mechanical or a bad connection. The only other thing might be a clogged cat. The loose stuff could settle to the bottom and then after the car has run for awhile, the junk might get blown around and clog the cat. i admit that this is pretty far-fetched.
