Remove the whole headlight in it's bucket/frame. Under the front of the hood there are two small bolts on top of each light. Remove these. There is on torx screw holding the assembly to the car on the inside of the engine bay (backside of the light assembly). Finally loosen (do not remove) the screw that holds the assembly to the side marker lamp. Then remove plug and viola...comes right out of the car.
DO NOT attempt to remove the lamps from the frame. They are held in with little plastic clips that snap into the frame and onto the lamp itself. They are brittle by now and BREAK easily and are a serious PIA to fix properly.
To clean the fogginess out of your lamps use as RS said...some rubbing alcohol (careful as possible not to let it contact the reflective material too much) or something like CLR. Rinse with water then alcohol again...then blowdry and re-install.
