How about:
- Intake manifold gasket?
- Fuel injector O-rings? (don't know how likely this is, but...)
I suppose it's easy for us to get so fixated on "vacuum lines" that we miss something else.
Have you tried the WD-40/carb cleaner inspection method? I haven't had great luck with it, but other people swear by it. Maybe someone who's made it work for them can suggest a technique.
I know you checked the vacuum line to the vacuum reservoir. On my car the check valve in the vacuum reservoir had gone bad, and was leaking air continuously. I was able to take the approx .75" cap off the reservoir (where the line to the manifold connects) and rebuild the check valve. If you already check this by applying your Mighty Vac to the line at the manifold, and the line holds a vacuum, then I guess you've eliminated this.
I'll keep thinking...
