A gear coming loose from a shaft points to a mechanic problem.
Having some experience in failure modes and rates, I would assume that it was a faulty motor - the gear wasn't attached to the shaft properly. It does happen. It is unlikley the load of the engine could be impacting it - I've seen plenty of starters trying to move a siezed engine and the gear stays on.
About all I can think of is a misalignment between the starter gear and the flywheel. On your old starter, and the starter that just failed - are there any unusual wear marks on the starter gear? Does it look like only part of the gear engages? That would be the only thing I could think of.
I'd say 75-80% chance of faulty starter, the remainder being a misalignment. The most common reason for a new part to fail soon after installation is a faulty part (called infant mortality in the business)
